NURSING HOME CONSULTATION PROGRAM

WHAT IS THE NURSING HOME CONSULTATION PROGRAM?

Our Nursing Home Consultation Program provides behavioral health assessments and monitoring for people that live in nursing homes who are struggling with mental illness and/or dementia. Our services are designed to assist the individual in adjusting to, and maintaining their in-residence care.

The Reliance Community Care Partners Nursing Home Consultation Program is offered for those residing in Kent County and is funded by Network180 and Medicare.

HOW DO I ENROLL IN THE NURSING CONSULTATION PROGRAM?

If you believe you or your family member would benefit from this program, please speak to a social worker at the nursing facility to get a referral.

Following a referral, an intake worker will contact you, your family, or your legal representative to offer services and answer any questions. Our staff member will assist you in completing any necessary paperwork.

Once paperwork is complete, a Reliance social worker will contact you to complete the assessment and begin services. If you have questions, please feel free to give us a call.

HOW WE HELP

Reliance provides the following Nursing Home Consultation services:

  • Mental health assessment & monitoring
  • Supportive visits from a social worker
  • Counseling
  • Behavioral Support
  • Medication monitoring & recommendations
  • Facility staff consultation

WHO IS ELIGIBLE?

Individuals who currently live in a nursing home, in Kent County and are diagnosed with a chronic and persistent mental illness and/or dementia.

WHO PAYS FOR THE SERVICES?

Services can be paid for through Medicare and Medicaid insurances. During the assessment process our intake worker will assist you in reviewing your insurance coverage. In rare instances an individual may be required to pay for part of the cost of services.
