Reliance has been a resource for elders, caregivers, and others with long-term care needs since 1979. Through a variety of activities, case managers coordinate the services that help with daily life and assure the appropriate level of gentle and dignified care for each individual’s need.
Reliance Community Care Partners is committed to maximizing the independence of long-term care participants & consumers, supporting loved ones through a range of services. Eligibility for program entry is different for each of our services.
