
Person-Centered Options Counseling (PCOC), through MI Options, is a free service to walk you through a complex system to create a long-term plan that fits your goals, values, and lifestyle. This program provides information to help individuals make informed decisions and exercise control over long-term care needs by incorporating their personal goals and preferences. This program can help individuals:
- · Explore different long-term care services, including help at home or in the community.
- · Learn what to do after a big life change and what services may be available.
- · Get support making a plan that fits your goals and what matters most to you.
This project has been made possible through a grant awarded to The Information Center by the Michigan Department of Health and Human Services.
PCOC is available to any Michigan resident 18 years and older, their family members, or caregivers who support them regardless of income, assets, or disability status. It is funded by MDHHS, at no cost to the individual.

You can enroll in this program by calling the Statewide Call Center at 1-800-803-7174 from 8 a.m. – 8 p.m Monday through Friday. You can also call Reliance directly at 800-447-3007 from 8:00 a.m – 4:30 p.m. Monday through Friday.
A referral can be made by anyone, whether it is the individual, a professional, friend, or family member.
Through Person-Centered Options Counseling, our Options Counselors can connect individuals to resources and services already available in your community and assist with any referrals and necessary paperwork. The Options Counselor will also follow up with the individual, as needed, to make sure needs are being met. These services are provided through:
- Personalized Planning: Trained Options Counselors help identify goals, strengths, and preferences to create a customized long-term care plan.
- Support for Independence: Explore home and community-based services such as in-home care, chore services and home-delivered meals to help individuals stay in their homes.
- Information and Referral: Connects individuals with local services and support for navigating complex care systems.

- Food Assistance & Home-Delivered Meals
- Home Modifications or Home Repair

Reliance assists those qualifying for the MI Options program who live in Alcona, Allegan, Alpena, Antrim, Arenac, Benzie, Charlevoix, Cheboygan, Clinton, Crawford, Eaton, Emmet, Grand Traverse, Ingham, Ionia, Iosco, Kalkaska, Kent, Lake, Leelanau, Manistee, Mason, Mecosta, Missaukee, Montcalm, Montmorency, Muskegon, Newaygo, Oceana, Ogemaw, Osceola, Oscoda, Otsego, Ottawa, Presque Isle, Roscommon, and Wexford Counties.
