WHAT IS FACILITY-BASED BEHAVIORAL HEALTH SERVICES?

Reliance Integrated Health Facility-based Behavioral Health team provides therapy and psychiatric medication consultation for people who are living in a facility and who are struggling with mental illness or dementia. Specialized services are provided by master-level Social Workers, Psychiatric Nurse Practitioners, and Psychiatrists who have been uniquely trained in the specialty of older adults and psychiatric assessment.

HOW WE HELP

Services are designed to assist individuals in adjusting to and maintaining in-residence care. Counseling, psychiatric assessment and medication management occur directly with the individual and include coordination of care with facility staff.  Reliance helps through the provision of the following Facility-Based Behavioral Health Services:

  • Counseling
  • Medication monitoring & recommendations
  • Facility staff consultation

WHO PAYS FOR THE SERVICES?

Services can be paid for through a variety of insurances. During the assessment process our staff will assist you in reviewing your insurance coverage.

WHO IS ELIGIBLE?

Individuals who currently live in a facility and are experiencing emotional distress, mental illness, and/or dementia.

HOW DO I ENROLL?

If you believe you or your family member would benefit from this program, please contact Reliance to speak to a social worker at your facility to get a referral; request Reliance services. Following a referral, an individual from Reliance will contact you, your family, or your legal representative to offer services and answer any questions. Our staff member will assist you in completing any necessary paperwork. Once paperwork is complete, a Reliance team member will contact you to complete the assessment and begin services.
