What Will You Do?

In this community-based program, the Social Work Case Manager provides case management services out of his/her home office for the MI Choice Waiver Program.  

Responsibilities

·         Both in-home and phone assessments to formulate plan of care

·         Coordination of care with other team members to develop, initiate, facilitate, and monitor the plan of care

·         Mobilize and coordinate family and/or friends to assist with services needs

·         Arrange and participate in person-centered conferences as needed

·         Evaluate and coordinate safety risk assessments in the participant’s intended residence

·         Advocates on behalf of participant in care related matters for procurement of services and entitled benefits

·         Identifies individuals who could benefit from disease management, screens, and offers MI Choice Waiver program

·         Provides education and empowers the participant in implementation of the person-centered care plan

Education and Experience

·         BSW or MSW (preferred) licensure in the State of Michigan

·         At least 2 years of clinical Social Work experience

·         Older adult care or experience working with disabled population preferred

·         Knowledge of community-based resources and strong networking skills

·         Certified Case Manager (CCM) is preferred

We are seeking a full-time Transportation and Medicaid Specialist to join our team of professionals in our Corporate Office to support the Case Managers in the field! This key support role works closely with our case management team and provider network staff assisting our participants in securing needed transportation services for the MIChoice Medicaid Waiver program (e.g. medical appointments, community integration). This position also works with our intake team in assisting potential participants with securing their Medicaid benefit.

Requirements for this position:

  • High school diploma or equivalent, Associate’s or Bachelor’s Degree preferred
  • Proven ability to organize and prioritize workload independently
  • Reliable vehicle, current valid driver’s license and proof of auto insurance
  • Proficient in Microsoft Suite applications
  • Exceptional interpersonal and customer service skills and follow through
  • Strong written and oral communication skills
  • Accurate and detailed documentation skills

Our preferred candidates will also be bi-lingual (Spanish), have knowledge of community resources for older adults and those with disabilities, expereince with CHAMPS, knowledge of long term care Medicaid and completing Medicaid applications.
