Description

Reliance Community Care Partners is seeking an RN Intake Evaluator for our various long term care programs.

Responsibilities:

  • Determine program eligibility
  • Schedule visits
  • Perform in-person participant assessments
  • Responsible for planning, implementing and coordinating care plans to facilitate appropriate services across the continuum of care
  • Provide referral management as needed to assist individuals with service program options

Job Qualifications:

  • Current, unrestricted RN license in the State of Michigan
  • Minimum of two (2) years of direct clinical experience
  • Preferred experience for this position: home care, long term care, and case management

Reliance Community Care Partners has a unique opportunity for a Social Worker to work as full-time Intake Coordinator. This position is based at the Corporate Office in Grand Rapids with some days in the office and some days at home. It offers the opportunity to work as an essential part of an interdisciplinary health care team.

What Will You Do?

The Intake Coordinator evaluates telephone inquiries for Reliance Community Care Partners programs to determine potential program eligibility and schedule assessments. This key member of the team provides assessment, education and referral management as needed for program participants.

Education and Experience

  • BSW or MSW licensure in the State of Michigan (full license preferred)
  • At least 2 years of social work experience
  • Older adult care or experience working with disabled population preferred
  • Knowledge of community-based resources and strong networking skills
  • Experience in providing quality customer service over the phone
