RN, Case Manager – MI Choice Waiver Program

Shift: Full Time, 1st Shift, Mon – Fri

Due to a well-deserved promotion, we are seeking a full-time RN Case Manager to join our team of health care professionals!!  The RN Case Manager is responsible for performing in-person participant assessments, which include planning, implementing, coordinating, monitoring and evaluation of case plans to facilitate appropriate services across the continuum of care.  This position will work primarily on the MI Choice Waiver Program out of his/her home office. 

This opportunity requires licensure as an RN in the State of Michigan and a minimum of 2 years of nursing experience working with individuals experiencing mental illness.  Experience with the older adult population and community management is preferred.
