Social Work, Case Manager – MI Choice Waiver Program

Shift: Full Time, 1st Shift, Mon – Fri

Reliance Community Care Partners is seeking a full-time Social Worker to provide case management services from his/her home office for the MI Choice Medicaid Waiver program.  In addition, to a love of working with seniors, this position requires licensure as an MSW or BSW in the State of Michigan (limited license is acceptable; full licensure with Master’s preferred) case management experience (CCM preferred), and a minimum of 2 years of social work experience in long term care.  Experience in home care and/or in community hospice is relevant and valued as well.  This position provides case management services in Kent, Ottawa and Allegan Counties.
