LMSW, OBRA Evaluator – 2-3 assessments per week

STATUS: Per Diem, 1st Shift, As needed.

Reliance Community Care Partners is seeking to add a per diem Social Work OBRA Evaluator to our team of stellar health care professionals.  The OBRA Evaluator is responsible for performing in-person assessments for individuals who may have been diagnosed with a mental health condition and/or developmental disability, and may require placement within a long term care facility. An assessment is completed to determine the level of care, as well as to make recommendations for both nursing and mental health services.

Top candidates will be fully licensed as an MSW in the state of Michigan as well as have at least 2 years of clinical experience including mental health experience.  Serious candidates must also have reliable transportation to make visits in the community.
