What Will You Do?

In this community-based program, the Social Work Case Manager provides case management services out of his/her local home office in the Traverse City area for the MI Choice Medicaid Waiver Program.

Responsibilities

  • Both in-home and phone assessments to formulate plan of care
  • Coordinate care with other team members to develop, initiate, facilitate, and monitor the plan of care
  • Mobilize and coordinate family and/or friends to assist with services needs
  • Arrange and participate in person-centered conferences as needed
  • Evaluate and coordinate safety risk assessments in the participant’s intended residence
  • Advocate on behalf of participant in care related matters for procurement of services and entitled benefits
  • Provide education and empower the participant in implementation of the person-centered care plan

Education and Experience

  • BSW or MSW (required) full or limited licensure in the State of Michigan
  • At least 2 years of clinical Social Work experience
  • Older adult care or experience working with disabled population preferred
  • Knowledge of community-based resources and strong networking skills
  • Certified Case Manager (CCM) preferred
  • Hospice and/or certified home care experience is highly valued

  • Provide leadership and supervision of clinical staff, approximately 25 nurses and social workers
  • Daily management of the MI Choice Medicaid Waiver Program
    • Budget, quality, and personnel
    • Adherence to regulatory and contractual compliance

Education and Experience

  • RN licensure in the State of Michigan in good standing
  • At least 2 years of relevant clinical experience, preferably in long term care or home care
  • Case manager certification is highly valued
    • If not certified, the expectation is that it would be obtained and maintained within 3 years of hire
  • Management experience as well as knowledge/experience supervising remote team members
  • Knowledge of the MI Choice Waiver Program

This position is based at the Corporate Office in Grand Rapids with some days in the office and some days at home. If offers the opportunity to work as an essential part of an interdisciplinary health care team.
