This position offers flexibility, work from home as well as out in the local community and the opportunity to work as an essential part of an interdisciplinary health care team. This role will cover 2-3 counties within West Michigan.

What Will You Do?

In this community-based program, the RN Case Manager provides case management services from his/her home office and surrounding community for the MI Choice Waiver Program.

Responsibilities

  • Both In-home and phone assessments to formulate plan of care
  • Coordination of care with other team members to develop, initiate, facilitate, and monitor the plan of care
  • Mobilize and coordinate family and/or friends to assist with services needs
  • Arrange and participate in person-centered conferences as needed
  • Evaluate and coordinate safety risk assessments in the participant’s intended residence
  • Advocates on behalf of participant in care related matters for procurement of services and entitled benefits
  • Identifies individuals who could benefit from disease management, screens and offers MI Choice Waiver program
  • Provides education and empowers the participant in implementation of the person-centered care plan

Education and Experience

  • RN licensure in the State of Michigan
  • At least 2 years of RN experience in direct clinical care
  • Older adult care or experience working with disabled population preferred
  • Knowledge of community-based resources and strong networking skills
  • Certified Case Manager (CCM) is preferred
  • Hospice and/or certified home care experience is highly valued

Quality Coordinator – Claims, full-time

Reliance Community Care Partners is seeking a full-time Quality Coordinator – Claims Representative to join our team of professionals

What will you do:

  • Perform claim audit functions and data management to ensure date integrity, record completeness, compliance with contractual and accreditation requirements
  • Work with program staff to facilitate data integrity and accuracy along with data management and analysis that includes submission and correction of data.
  • This key team member has the option to work in the Corporate Office or in their own home office setting (once oriented).
  • The schedule is Monday-Friday, 8:00am – 4:30pm.

Qualified candidates:

  • High school diploma or equivalent
  • Minimum of 1 year of experience in medical claim processing
  • Top candidates will be skilled in efficient and accurate, detailed data entry
  • Experience with claims processing software, and knowledge of medical terminology and ICD10, CPT, and HCPCS coding is preferred

Social Work Transition Navigator

The Transition Navigator offers flexibility, work from your home office, traveling to participant locations in central and southwest Michigan and access to a team of other experience navigators.

What Will You Do?

In this community-based program, the Social Work Transition Navigator assesses individuals in an institutional setting and assists with overcoming barriers to allow for a safe transition into the community setting of the individual’s choice.

Responsibilities

  • Perform in-person assessments to identify the individual’s desires, resources, strengths and barriers to transitioning into the community
  • Formulate an individualized plan of care to facilitate the transition
  • Coordinate with other natural supports and professionals to develop, initiate, facilitate and monitor the plan of care
  • Find housing options and coordinate with apartments, assisted living facilities, etc.
  • Conduct or coordinate home safety evaluations
  • Advocate and refer individuals for procurement of services and entitled benefits
  • Follow the individual in the community for as long as the individual is eligible
  • Participate in shopping for various needs
  • Documentation in different software systems
  • Reliable transportation to travel throughout various counties

Education and Experience Requirements

  • BSW or MSW licensure (limited or full) in the State of Michigan (Master’s preferred); or
  • Have a bachelor’s degree in a health or human services field and at least 3 years of experience in the provision of health or social services; or
  • A Community Health Worker certification with at least 3 years of experience in the provision of health or social services
