What Will You Do?
In this community-based program, the Occupational Therapist OBRA Evaluator is responsible for performing in-person assessments for individuals who may have been diagnosed with a mental health condition and/or developmental disability and may require placement within a long-term care facility. An assessment is completed to determine the level of care, as well as to make recommendations for both nursing and mental health services.

Education and Experience
- OTR licensure in the State of Michigan
- At least 2 years of clinical experience, including mental health experience

Description
Reliance Community Care Partners is seeking an RN Intake Evaluator for our various long term care programs.
Responsibilities:
- Determine program eligibility
- Schedule visits
- Perform in-person participant assessments
- Responsible for planning, implementing and coordinating care plans to facilitate appropriate services across the continuum of care
- Provide referral management as needed to assist individuals with service program options
Job Qualifications:
- Current, unrestricted RN license in the State of Michigan
- Minimum of two (2) years of direct clinical experience
- Preferred experience for this position: home care, long term care, and case management
