This position offers a Monday – Friday, daytime schedule with flexibility, working from home locally in the Kent County area as well as daily visits out in the greater Grand Rapids community and the opportunity to work as an essential part of an interdisciplinary health care team.

What Will You Do?

In this community-based program, the Social Work Case Manager provides case management services out of his/her home office for the MI Choice Waiver Program.  

Responsibilities

·         Both in-home and phone assessments to formulate plan of care

·         Coordination of care with other team members to develop, initiate, facilitate, and monitor the plan of care

·         Mobilize and coordinate family and/or friends to assist with services needs

·         Arrange and participate in person-centered conferences as needed

·         Evaluate and coordinate safety risk assessments in the participant’s intended residence

·         Advocates on behalf of participant in care related matters for procurement of services and entitled benefits

·         Identifies individuals who could benefit from disease management, screens, and offers MI Choice Waiver program

·         Provides education and empowers the participant in implementation of the person-centered care plan

Education and Experience

·         BSW or MSW (preferred) licensure in the State of Michigan

·         At least 2 years of clinical Social Work experience

·         Older adult care or experience working with disabled population preferred

·         Knowledge of community-based resources and strong networking skills

·         Certified Case Manager (CCM) is preferred

We are seeking a Clinical Therapist to join our team of mental health care professionals!! The Clinical Therapist is responsible for providing individual and group psychotherapy services for adults living in the community, ages 18 and older with a specialty for older adults, with mental illness or co-occurring disorders of mental illness.

This opportunity requires licensure as an MSW in the State of Michigan and a minimum of 2 years of direct clinical experience, including mental health experience. Our Clinical Therapist will independently conduct an assessment as permitted within the scope of practice of the discipline. Previous therapy experience is highly valued.

In this community-based program, the RN OBRA Evaluator is responsible for performing in-person assessments for individuals who may have been diagnosed with a mental health condition and/or developmental disability and may require placement within a long-term care facility. An assessment is completed to determine the level of care, as well as to make recommendations for both nursing and mental health services.
