KENT COUNTY SENIOR MILLAGE SERVICES
WHAT IS CARE MANAGEMENT AND HOME SUPPORT?
Care Management and Home Support are services that are designed to find, set in motion and manage a variety of homecare and other services needed by frail, elderly and/or disabled persons aged 60+.
WHO IS ELIGIBLE?
Anyone 60 years of age and over who:
- Resides in Kent County
- Meets income and asset eligibility
- Ability to perform individually or live independently is limited
- Needs help with many of the usual daily activities such as bathing, cooking, shopping, cleaning, etc.
Funding provided by KCSM and administered by AAAWM.
PROVIDER NETWORK
Reliance Community Care Partners has an extensive Provider Network that encompasses the full array of services in Kent County covered by the Senior Millage Program. Each Provider goes through a credentialing process and is monitored to assure quality services and compliance with requirements. Reliance Case Managers work closely with Providers to assure that each individual receives the services they need as identified in their person-centered plan. Providers are added to the Network based on service needs.
HOW WE HELP
We help plan the required care, using informal and formal support and resources, to help eligible older adults remain living in their own home.
With follow-up and monitoring, we assure that the necessary services and supports are in place as planned and that changes in needs are identified and addressed.
HOW DO I ENROLL?
Call Reliance and tell us you are interested in the Care Management and/or Home Support programs. When you call, we will ask you several questions to see if you qualify. We will assist you in finding resources for help. Not everyone who calls will be eligible for services. If you don’t qualify, we will suggest other resources.
A comprehensive assessment will be conducted by a case manager or care management team consisting of a registered nurse and/or social worker. You approve all services, and only the services necessary to enable you to remain at home. To find out more about these programs, call 616-954-1573.
