WHO IS ELIGIBLE?
Our services are suited for adult individuals and couples. We will work with your insurance to make sure your care does not add to your concerns.
HOW WE HELP
In partnership, our staff will design a plan that will assist you in developing and polishing the strengths and skills you may not know you already possess. Counseling therapy with trained social workers can be a first step to exploring your personal story and seeing it from a new perspective – one that includes plans for the positive changes you are ready to make. Together with our Reliance team, we will find an approach that matches your needs.
Sometimes, therapy combined with medication assistance is recommended. Our Physician Assistant is certified in Psychiatry. He will make recommendations based on a thorough assessment starting with your view of what you think will help. With you at the center, our PA will work closely with our nurse to ensure that your overall wellness remains our primary concern. Our nurse can provide health education and will work with your doctor to ensure all care is coordinated.

WHAT WE DO
We work collaboratively with health care providers, government agencies, Insurance providers, legal representatives, and family members to make sure those we serve receive the attention and care they need to live healthy, meaningful, and productive lives.
WHAT IS BEHAVIORAL HEALTH CASE MANAGEMENT?
A program funded by Network180 that provides case management services for individuals living in Kent County with aging-related behavior health issues, severe mental illness, and/or a co-occurring substance abuse.
WHO IS ELIGIBLE?
Network180 determines eligibility for our Case Management program. Eligibility is based on the age of an individual; presence of aging-related behavioral health issues; the mental health diagnosis; how the symptoms of mental illness impacts the individual, and whether case management services would be helpful. The level of insurance coverage does not affect eligibility.

We encourage recovery by promoting psychiatric stability and helping to establish a more meaningful daily life by providing support and opportunities for personal development.
HOW DO I ENROLL?
Contact Network180 at (616) 336-3909 to
schedule an appointment to determine eligibility. Once eligibility is confirmed, Network180 will refer you to Reliance. One of our case managers will visit you in your home or a location of your choosing to complete the necessary paperwork and begin working on a plan of care.
