WHAT IS A LOCAL CONTACT AGENCY?
Reliance Community Care Partners serves as a Local Contact Agency (LCA) for all current nursing home residents who desire to receive their long-term care in a private residence.
WHOM DO WE HELP?
Reliance assists residents who live a nursing facility located in the following counties:
Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Muskegon, Newaygo, Oceana, Osceola, and Ottawa.
HOW WE HELP
We’re here to help provide alternatives to long-term care in a nursing home and initiate referrals to facilitate successful community- based living, based on the choices of the resident.
Residents who are interested in other community living arrangements outside of the nursing home simply need to respond “yes” to Section Q on the MDS form. We will contact you within three days from receipt of the referral to set up a formal meeting to discuss your eligibility and options. Family, friends, or staff members from your facility are welcome to join us in the meeting. To find out more about this service, call 616-954-1573.

We work collaboratively with health care providers, government agencies, Insurance providers, legal representatives, and family members to make sure those we serve receive the attention and care they need to live healthy, meaningful, and productive lives.
